An In-Person Event

Transitions to and from an acute care setting can be difficult for those living with dementia. These transitions can cause challenging times, and communication is critical. Our goal is to create a smooth transition between home, community, and SNF. In this program we will discuss how timing, challenges and the impact of dementia can the smooth transition of the person living with dementia.
