At Hartwell Place, you can be part of a caring community where all programs and events are designed with you in mind. With just 28 apartments, Hartwell Place is a warm, intimate home exclusively for those with memory loss.

Alzheimer's CareThis residential setting on a quiet neighborhood street offers a warm and intimate home-like setting with suites. A high caregiver-to-resident ratio enables us to stay focused on every resident’s unique needs. With extensive, personalized programming designed for people with memory loss, we work to bring out the best in everybody’s abilities and life enjoyment.

To ensure that each resident feels comfortable and secure with our services, we partner with their families to personalize service plans for every individual. We take the time to explore personal history and interests so we can tailor services care for each unique need. By monitoring and modifying plans, we remain responsive to changes.

Provided by a staff trained in dementia care, our services encourage residents to fully use their abilities to make choices and maximize their independence. We support their well-being with safe, stimulating, multisensory opportunities that focus attention and foster self-esteem while reducing anxiety.

Through our activity based programming we focus on stimulating mental, social and physical engagement. Every day there are at least two structured activities offered per hour to accommodate individual interests and abilities.

A Life Story Assessment is completed for each resident. This includes important facts about the person’s life, their interests and hobbies, their career or leisure experiences as well as their favorite foods, movies, etc. This tool allows us to build the content of our activity programming around the specific interests of each resident.

We put an emphasis on community engagement as well – daily walks in the neighborhood and frequent outings to museums, entertainment, special events, shopping and dining are just a few of the opportunities we offer.

For those whose needs expand, priority access to CMSS services, including short-term rehab and advanced memory care at Wesley Place are always available.
